ADOPTION ELIGIBILITY QUESTIONNAIRE

The aim of this questionnaire is to initially assess your adoption eligibility.  Please answer each question below as clearly as possible. Provide as much detail in your responses as you think necessary and be truthful.

 

Once we receive your submission, if we feel you are potentially eligible, we will forward the full Adoption Questionnaire for you to complete. We will then make a final decision.

 

All information provided will be kept strictly confidential within Blue Weimaraner Rescue & Rehoming UK, and will not be shared.
